HDPE sheets cutting

I am looking for a shop(in US or better in SF Bay Area) who can cut 0.01 thick(or thinner) HDPE sheets to rectangular shape (2'x3") and put a couple of creases lengthwise or thermoform it into U shape. I would order 3000 pieces.
