My bet is that they were hit by a train!

Just yesterday, someone was killed contacting the 600 VDC third rail of a Boston subway line. Cause of death was cardiac arrest.

One particularly painful incident occured a few years ago at the Kedzie grade level "L" crossing in Chicago. A very stupid man decided to relieve himself on the third rail.
