In the balloon properties, set the balloon text to custom, then paste this into the custom text field:
$PRPMODEL:"SW-File Name"
This does not include the file name extension (.SLDPRT, .SLDASM). I don't know of a way to get those autmatically.
You can find more of these by exploring the link button on a note annotation.
For me that syntax doesn't work. The one below works fine though:
$PRP:"SW-File Name"
Also, if you right-click your note, choose Properties and click 'Link to Property', in the dropdown-box you see some other properties you can link to.

Unfortunately, you have to set up the balloon properties every time you start the tool.
I agree that this is something that should be controlled in the document properties. I'm off to cast a vote for it via enhancement request.
I have not tested this but couldn't you store that as an annotation favorite & then drag & drop it from the Design library when needed? I am speaking as in 2005. In 2004 you might just use a copy & paste method. Create one & the copy it to other faces (no need to reenter it over & over). Lets us all know if that works

You can enter the "formula" when you get into the balloon command and all of the balloons that get placed, while still in the command, will use the "formula".
Almost automatic :)
