Okay, so I'm late and catching up, but Gunner wrote on Fri, 29 Sep 2006 16:16:30 GMT in rec.crafts.metalworking :

I talked with the supervisor the other day about scrap tags (big push to keep the scrap rate down. Each bad part gets a scrap tag, which lists what's wrong, what was the cause, and the corrective action taken.) I told him that I can figure out what _I_ did wrong, but looking at someone else's screw up and I often have no clue as to "how did they do that?" I mean, I can tell you exactly why that last part came loose, super came up and we started talking about a detail, and when he was done with the fix, I punched cycle start, having forgotten the itty bitty minor detail of tightening the 5 10x24 cap screws. "oops! - I hate it when that happens!"
