Hayseuss Freakin' Crisco. Just yank the damned thing out, rinse with povidone rinsing solution, and bondo the hole. It'll heal in no time.
Those bolt cutters likely sent splinters of hardened file all over the guy's skull and scalp while fracturing the skull worse.

Indeed. I was rather shocked at that "treatment". It was..moronic
A straight pull out..vacuum the wound well, treat with antibiotics...install a plug and voila
