When I access the feature Hole wizards, my SW2004SP4 Crash. I´m using Windows 2000. The message that appear is something wrong with table Defaults.mdb, but I already change the file and the problem continues.
Somebody, have any idea what is happen?

Well, just a wild guess, but maybe the problem is that you've changed your default.mdb? I'd bet you lunch that the problem goes away if you reinstall or repair your installation.
